Appointment booking regulations

Patient registration can be made in person (by the patient, a family member, or a third party), by phone at +48 616 394 040 or +48 570 991 333, by email, through the registration form available on the website www.dentysta-royaldent.pl, via the online booking systems IQ Dental or Znany Lekarz, as well as through social media (Facebook or Instagram), with an agreed date and time for the provision of dental services.

All appointments are confirmed by the clinic by 3:00 PM on the previous day via SMS or phone call. If there is no contact with the patient by 3:00 PM on the previous day, the appointment may be cancelled.

Failure to cancel an appointment at least 24 hours in advance is considered a missed appointment without notice. If a patient fails to attend an appointment without prior cancellation, they will be moved to the end of the waiting list. To book another appointment, a patient who missed their previous visit without notice is required to pay a deposit of 50% of the planned procedure cost, by bank transfer to account number: 14 1750 0012 0000 0000 3681 0963, in person, or via BLIK, at least 3 days before the planned visit.
This amount will be deducted from the cost of the next or final appointment.
If the patient fails to attend again without notice, the deposit is non-refundable.

Admitting a patient requires maintaining medical and statistical documentation.

On the day of the appointment, the patient is required to present an identity document (e.g., ID card, passport, or driver’s license).

This obligation arises from the need to verify the patient’s identity for the proper management of medical documentation, in accordance with:

  • Article 25(1)(1) of the Act of 6 November 2008 on Patients’ Rights and the Patient Ombudsman (Journal of Laws of 2023, item 1545, as amended), and
  • §10(1) of the Regulation of the Minister of Health of 6 April 2020 on the types, scope, and templates of medical documentation and the manner of its processing (Journal of Laws of 2020, item 666).

Presenting an identity document allows for accurate data assignment in the system and ensures the security and protection of medical records.
